UHHO must verify income eligibility
SERVING: Houston Harris & Fort Bend County
TIME: 12pm - 3pm
LOCATION: 2703 Highway 6 South, Suite 145, Houston, Texas 77082
Please bring the following documents to be serviced:
1. ID for all people in the household 2. Proof of residency (utility bill, lease or mortgage contract, landlord statement, etc.) 3. AND any of the following will be accepted for proof of income (all income must be disclosed): - Pay Check Stub - Medicaid or Food Stamp Documents - WIC Documents - Unemployment Insurance Paperwork - 1040 forms - Pension retirement cards or statements
Individuals and families must also establish their eligibility by a signed self-declaration of income (provided by UHHO). |
